Patient notice of financial assistance: Circle Family HealthCare Network is proud of its "Not-For Profit" mission to provide quality care to all who need it regardless of ability to pay. CFHCN will treat all patients equitably, with dignity and compassion. If you do not have health insurance and worry that you may not be able to pay in full for your care, we may be able to help you. Circle Family HealthCare Network provides financial assistance to patients based on their income, assets, and needs. In addition, we may be able to help you get government provided health insurance or work with you to arrange a manageable payment plan. It is important that you let us know if you will have trouble paying your bill. Uncooperative patients may be turned over to a collection agency. This could affect your credit status. Patient responsibilities: To be considered for financial assistance the uninsured or underinsured must cooperate with our Financial Counselor to provide the information and documentation necessary to apply for other existing financial resources. To be considered for a discount under the financial assistance policy, an uninsured or underinsured person must provide the organization with all financial and other information needed to determine eligibility.
