Dr. Andre Hines, DHA

Chief Executive Officer

Circle Family HealthCare Network (CFHCN) welcomes André Hines, DHA as its new Chief Executive Officer (CEO).  Dr. Hines is an experienced senior healthcare administrator with over 15 years of service in healthcare administration, program management, strategic planning, and fiscal management.   

 

Dr. Hines is excited about her return to the Westside of Chicago as all of her experience prior to Alaska has been serving Westside communities and its residents.  Dr. Hines passion is having the opportunity to work with a faith-based organization to deliver holistic care to our underserved communities.  Working with CFHCN gives her opportunity to mentor to people’s spiritual needs as well as their physical, mental, and emotional needs.  After 10 years, Dr. Hines continues to embrace her Austin community roots, where she continues to serve individuals and families mind, body, and spirit through medical health, behavioral health, and social services.

Dr. Hines received her Doctor of Health Administration from Central Michigan University in Mt. Pleasant, MI.  She completed the MATCH Postgraduate Fellowship in health care administration at Johns Hopkins University & The National Association of Community Health Centers in Baltimore, MD, and a Postgraduate Fellowship with Aquinas Institute of Theology & Ascension Health in St. Louis, MO.  She secured a Masters Degree and a Bachelors Degree in Public Administration at Roosevelt University.  Currently, she serves on the Board of Directors for the Christian Community Health Fellowship (CCHF). 

 

Dr. Hines tenure has included a diverse background in community leadership, community organization, and philanthropic planning and management.  Over the last decade Dr. Hines has provided healthcare administration and support service programs to diverse populations.  Most recently she was the CEO for a frontier extended stay community health center in rural Alaska, where she was responsible for an outpatient community health center, in-patient services, and a pharmacy.  In 1995, Dr. Hines served Circle Family Care as its Vice President of Health Services.

Bruce Peoples, M.D.

Chief Medical Officer

                             

Dr. Peoples is a dedicated physician who has a strong commitment to serving the community and its residents. His expertise in the area of Family Health is exceptional. He has compassion for his clients and is a source of strength to their families. Dr. Peoples is dedicated to providing exceptional health care to all.  Dr. Peoples held the position of Director of Senior Services at Mount Sinai Medical Center before joining Circle Family Care 5 years ago. In November 2006, he was appointed Interim Medical Director, and he was appointed Medical Director in July 2007. He is presently on the medical staff of West Suburban, Oak Park and Mac Neal Hospitals. He is a member of the American Medical Association, American College of Physicians and the Illinois State Medical Society. Dr. Peoples is a practicing Internist at Circle Family HealthCare Network.

 

 

 

 

 


Patrick C. Nwaezeigwe, MBA

Chief Financial Officer

                                                
Mr. Nwaezeigwe has worked in various levels of management, and has proven to be a result-oriented professional. His level of dedication and attention to detail will help move CFHCN to the next level. Strong experience in direct retrieval of lost revenue, preparation of financial statements, reconciliation of revenue and expense, coordination of internal audits, and preparation of monthly and annual budgets, has already paid huge dividend.

 

 

 

 

 

 

 

 


Philip Foust

Director of Operations

                               
Phil Foust has come “full-circle” to Circle Family HealthCare Network (CFHCN) after working here for 9 years, having left in January 2002 and returning in March 2010 to serve as Director of Operations. Phil Foust  is responsible for Human Resource Management as well as the key administrative resource for the organization including, Corporate Compliance, Risk and Safety Management and HIPPA.

In the late 1990’s Mr. Foust was instrumental in helping CFHCN (then Circle Family Care) become accredited through The Joint Commission on the Accreditation of Healthcare Organizations (JCAHO) and co-produced the organizations’ policy and procedure manual and develop a comprehensive employee training and information manual. Phil also oversaw the Fundraising and Development Department at that time producing a spring event and fall gala annually.

 

After a two-year hiatus in producing special events through his own business, Phil recently came back to us from serving as the Chief Operating Officer of a suburban post-prison, not-for-profit organization, which helped ex-prisoners transition back into society through the help of the local church, community and mentorship. Currently, Phil oversees the Operations Department, Human Resources, Safety and I.T. for CFHCN and serves on the senior management team.

 

Serving on the Westside of Chicago in an organization that provides access to healthcare for the underserved population is a passion of Mr. Foust and he is grateful for the opportunity to be part of the team at CFHCN once again. Phil states, “For me, I am reminded of God’s command to bind up the brokenhearted as the prophet Isaiah states in Isaiah 61:1, “The Spirit of the Lord is upon me, because the Lord has anointed me to bring good news to the afflicted; He has sent me to bind up the brokenhearted, to proclaim liberty to captives and freedom to prisoners…’ – New American Standard Version.

 

“I see my direct customers as the front-line staff persons of CFHCN. My job is to provide the administrative functionality and supply them with the tools they need to meet the needs of the Westside communities CFHCN serves. By doing this, I am helping to bring good news to the afflicted and bind up the broken hearted.”

 Mr. Foust has two adult children, three granddaughters and resides in Northwest Indiana with his wife, Julia. Together, they are committed to their local church and to serving others through the non-profit the community health center model.

 

 

 

 

 

 

Leslie J. Green

Director of Community Health Services

Meet Rev. Leslie J. Green, a new Circle Family member. Leslie is a bi-vocational professional dedicated to ministry and senior healthcare.  Leslie is an ordained clergywoman who speaks and ministers in her local church of 2,500 members, the Illinois District Council and denominational leadership conferences.   

 

 

She is a licensed nursing home administrator and dementia care specialist who was Administrator of Extended Care (132-unit licensed skilled and assisted living facility) at Renaissance Gardens at Sedgebrook and member of the Sedgebrook Executive Team (Lincolnshire, IL).  

 

 

Leslie is a faculty member of Rush Alzheimer’s Disease Center and Life Services Network.  She holds a Bachelor of Science in Healthcare Leadership from National-Lewis University (Wheaton, IL), a Masters of Art in Ethics and Pastoral Care from Garrett-Evangelical Theological Seminary at Northwestern (Evanston, IL) and has pursued studies in the Doctor of Health Administration Degree at Central Michigan University (Mt. Pleasant, MI). She served as the World Vision Co-Chair of the Chicago Vision Council for five years. And she is a member of the Professional Training Institute, Alzheimer’s Association – Greater Illinois Chapter.
