Clients Served * Adults * Adolescent * Children Community Health Services: This unit provides families and their children with medical care at four Federally Qualified Health Center (FQHC) community sites. CFHCN’s health center sites are located at 4909 W. Division St., 1633 N. Hamlin Ave. and 115 N. Parkside Ave., and 231 N. Pine Ave., Chicago, IL. Our medical services are delivered by caring physicians, nurses, nurse practitioners, a mid-wife, physician assistants, counselors, case managers and patient support specialists who believe in providing holistic care. Our staff responds to the needs of the community immediately and personally. Our providers are interested in building long-term doctor-patient relationships and offer expert, compassionate care to individuals at every level of need. Children and Family Services: This unit provides behavioral and mental health outpatient services to children (aged five to eighteen) and their families in the Austin, East and West Garfield Park, and Humboldt Park communities. We follow the best practice models that recognize that the family is the primary support system for the child. The family must participate as a full partner in all stages of the decision-making, treatment, planning process, including implementation, monitoring, and evaluation. Our primary goal is to strengthen family units. Community members may go to our site located at 4909 W. Division, Chicago, IL to receive quality behavioral healthcare. Behavioral Health Services: This unit provides services to adults in Humboldt Park at our 1633 N. Hamlin, 1629 N. Hamlin, and 3919 N. Albany, Chicago, IL locations. Clients who have mental illness, emotional disturbance, or have significant loss of normal functioning may receive services, which include psychiatric treatment, case management, individual, family and group therapy and housing assistance. Additional services are provided to victims of domestic violence and academic support to teens. School Based Health Services: This unit provides services to School Based Health Center (SBHC) services to schoolchildren and their families, in the Austin community, at our Austin School Health Center (ASHC) located on the Austin Campus, 231 N. Pine Ave., Chicago, IL. In partnership with the Chicago Public Schools (CPS), CFHCN is providing behavioral health and primary health services to high school students attending Austin Business & Entrepreneurship Academy, Austin Polytech Academy, and The VOISE Academy. In addition, services include health outreach and prevention education.
